Indigenous woman died in police custody after notification service not used - Her death is the first in the 16-year history of the CNS, which is supposed to be used to contact the Aboriginal Legal Service every time an Indigenous person is arrested. Instead, it was a month before the ALS was notified the Aboriginal woman Rebecca Maher had died in custody.
